Hub breaking issues. PLEASE HELP LOL

Alright so I picked up this explorer and it had auto hubs. I bought manual hubs and spine nut sets with washers for the swap. Everything has been installed correctly (been working on TTB axles for years) but for some strange reason I keep blowing my passenger side hub. Thought maybe my shafts were bent or I had a diff problem so I swapped all that out with others I had laying around. Checked my clearance with the camber bushings and the hole in the beam has been modified for more flex. Non of that seems to be the issue. I'm really stumped with this and wondering why it's only my passenger side? Any help will be appreciated as I come to this site on a daily bases and see the all the knowledgable folks on here.
 
What exactly is happening to the hub. Description would help but picture would be better.
 
I have a similar issue with the passenger side. It seems to eat the axle spacer washer that you put on before the C clip and it also eats the inner splines up on lockout hubs.

Check that your passengerside slip-yoke shaft has proper lubrication... The boot over it doesn't always seal 100% and needs lubed with CV-joint grease every so often (I usually do mine at the same time as the spindle & wheel bearings since it's just a simple matter of yanking out the shaft at that point).

Additionally you can try swapping to the later 3-piece splined washer (spacer) design with the Delrin plastic center ('95-'97 axles).
