I ordered them on Ebay. They cost me $19.00. I believe the seller has more listed. Type in Hi-lift mount and see what ebay brings up.

Thats a cool place to mount the jack. After much thought and the purchase of the mounts posted above I am thinking of mounting mine using my factory luggage rack. I think I finally found a use for it after 15 years of ownership.

Scrub the luggage rack idea! I ended up installing my jack on the spare tire carrier using the mounts I bought on Ebay. They worked great! I will post some photos later today,

I like them mounted on the tire carrier, out of the way. I wouls take the wingnuts off and put a regular nut on less chance of some one lifting your jack on you.
