Lean codes are really fun. All it means is that the HEGO pin reads under 0.5V with respect to the PCM ground, as the PCM commands the engine rich. Any reason you can think of that might do that (and there are a lot of them) works. Remember that the sensor will read lean if it detects oxygen, regardless of how it might have gotten there or whatever else might be in the exhaust (including fuel).
