What model transmission do I have?

should be a sticker on the transmission, if 26 years haven't worn it off.

You can also look at the door tag and get the trans code.
 
1995-2000 3.0l V6 will have a 4R44E 4-speed
2001-2008 3.0l V6 will have a 5R44E 5-speed

1998(1995-2000) 3.0l V6 can use either 4R or 5R as a replacement but it will always act as a 4-speed
A "5th" gear wasn't physically added to a 5R, it was a software change in the engine/trans computer
Internally the 4R and 5R are EXACTLY the same transmissions, the 5R got 2 new External sensors in 2001, which are not needed by the 1995-2000 engine/trans computer, so are simply left unplugged

A 2001-2008 3.0l can only use a 5R as a replacement
