I've never seen a crew cab one, but there were a option on 11th generation '04-08 F-150s - there's a couple SuperCabs around town here with that box.

Ford brochures say option on the crew cab from late '06 to '08. Wiki says a few '09s (12th generation) got that box as they used up remaining inventory.

Crew cab with 6.5' is rare on its own, this is almost unicorn territory. Add a 4.6 and manual - it would be.
