95 Belt Coming off and radiator fan not working

Hey just wanted to see if my assumption is correct, i just recently replaced my water pump and as i put everything back together and went to start the truck the serpentine belt spun for a second or two and then just lost track and fell off, also noticed my radiator fan is not spinning when engine is running. Could it be a bad fan clutch?
 
Check the routing on serpentine belt
All pulleys either have a ribbed surface or a smooth surface to match the side of the belt they use, smooth or ribbed

So if you have the smooth side of the belt on a ribbed pulley then it is wrong, no exceptions

Fan should still spin a bit even with failed fan clutch, more likely the water pump pulley was not spinning

Shows ribbed and smooth, only idler pulleys are smooth
 
all ribs were facing the ribbed way on the pulleys and the smooth surface was underneath the idler, so it comes down to the water pump pulley?
 
Thats what the fan is attached to, if it spins, fan spins

With engine off you should be able to move the fan by hand, won't spin free but can be moved easily
 
Okay but how would i be able to figure out what is throwing the belt off? Could it possibly be old belt? Because my pulley spins and the fan spins as well
 
Try looking across it from the side view and sight them all in together to see if one of the pulleys is not in line with the others
 
How does the belt tensioner feel? Pretty stiff? If not, then the tensioner might worn out.

Did the belt squeal while still in position? If it did, that would indicate a seized pulley.

My guess is something isn’t lined up.

If belt comes off when engine is running then a pulley is out of alignment, or as said belt was not tight
 
Make sure its properly in the grooves around the crankshaft pulley. Thats an easy one to miss as its not easily visible.
