94 ranger 3L auto reg cab short box.Warn man hubs.Took it in for safety insp.New ft rotors.Well Had it in on J-stands rt ft wheel play.Took apart, outer nut loose.No keway wedge in.Other side either ? Is this norn or did mech.leave it out or what ? Drivers side seems fine.Pass side when I tighten outer nut it also turns inner nut ?The only thing keeping outer nut tight is torque. Its a dana 28. confused.Thanks for any in put.
 
First off, that should be a Dana 35 as they discontinued the Dana28 with the 2.9's. Second, with lockouts, you have an inner nut, a lock washer w/ a tab on it and the outer nut. Inner nut is torqued to 16 INCH lbs. Outer nut is torqued to 150 FOOT lbs or more.

What about the '94 3.0 4x4?
 
A '94 will have either a D35 or the D28/35 hybrid axle. In either case the outer locknuts are the same.
If the inner nut is turning with the outer, then there's an issue with the locking washer between the two nuts (either it's missing, or it's tab has ridden up out of the spindle groove onto the threads).

Actually they discontinued the "True" D28 at the end of November 1989 about 1/3 of the way in to the 1990 model year.

The D35 replaced it.

But starting in 1993 Ford actually came out with a "Hybred" D28/35, which DANA referes to as a "D28" and created massive confusion.
