f-350 clutch problems help???

have a 2000 f-350 superduty. manual trans 7.3L D. problem is the clutch started to slip it felt like. then when i stopped and tried to reverse i pushed in the clutch and the peddle hit the floor. i checked everything up to the slave cylinder everything was working as it should. has anyone ever heard of this type of symptoms??? :icon_confused: the clutch was replaced 2 yrs ago. anyone have any idea???

Sounds to me like the clutch fork might have come off its perch. It happened to my ranger and my stang. Otherwise maybe air in the line, I dont know other than that.
 
if that was so would i be able to push the clutch fork in with my finger? with the slave cylinder off. have you ever see clutch forks wear down or break?
