The small diameter adjuster is wrong for the truck and would never self adjust, it looks like someone jury rigged it on a day when the parts stores were closed. I would get the right adjuster and assemble the brakes properly so you can depend on them, even if it means replacing more parts- shoes for example. It's cheaper than losing your brakes.
