92 buick lesabre whines on acceleration

sisters car having problems again. it used to backfire and slip till the filter got changed. ranfine for 2 or so months. she says it whines on acceleration. it doesnt do it when its revved up in park. and its rpm dependant. says itll get worse till it shifts then itll get better.

idk much about autos. other than that there are few great ones. car has like 230k on it tho.

Reads like 1st gear in the transmission has a problem, might be fixable with just a service, pan off, new filter and fluid.
But might need a complete rebuild.
It will only get worse over time so best to find out sooner than later.
