I have seen quite a few snapped spindles, the ones I have seen (impact/overloaded related) have always snapped inboard of the inner bearing so the axle stub and both sets of bearings stay with the hub.

Never seen one break between the bearings.

In theory, theory and practice are the same. In practice, they are different.
Perhaps, if overtorqued and no nut locking device installed, the inner race might start rotating with the wheel and make the nut spin with it, further overtorqueing the nut....

Sounds far-fetched. But I've seen and heard of stranger things. You would think the threads would strip.
