Unless you just want your oil filter to hit the manifold/header I'd have to say yes that adapter is required. I know with my headers it wouldn't fit sans the adapter.

I'm wondering if the oil filter being directly on the block sideways would cause dry startups so to speak cause it might empty out being sideways??

It should work as a filter would in any other sideways installation. It won't go back into the Ranger sideways; you'll need the adapter.

A filter that hangs down like the stock setup is the best way to avoid dry starts, otherwise the oil has much more tendency to siphon/drain back to the pan...not good. I don't care who's filter or what, that is my experience.

Run a good filter that has anti drain back (like a motorcraft, not a fram!) and it's not an issue.
