Need a new ECU/PCM/ECM

2004 Ranger 3.0 (gas) 2wd 5spd
Truck needs a new computer, I know I need to get same part number (4L5A-12A650-ALB XBT1).
My question is, other than PATS, does it have to be reprogrammed with the vin in order for the truck to run?
Also is it worth it to buy used or to buy the $200 reman that comes with vin programmed? Or are those a rip off?

Why do you think it needs a new ecm? Its very rare to have on go bad.

It ended up not being the pcm, it was the crank bolt backed off.
