I'm in a similar situation swapping a manual 1354 in place of an electric one. I have a complete 1350 manual transfer case and shifter sitting around I can rob the shifter off of but where can I find a shifter boot/bezel? I'm ok with using a universal boot if it comes down to it. Does the floor pan have an access panel I can remove or do I need to cut a hole?
Manual t-case shifter boot bezels that go together with the M5OD trans are pretty rare (the M5OD shifter position is farther forward than on earlier trucks where the two shifters together were more common). But after 1997, they are (almost) hen's teeth (only the coveted 2002-2003 FX4 trucks with the Torsen rear diff had them).
You might maybe try
car-part.com, search for a 1990-1997 BW1354 manual transfer case, then call up each yard that has one and see if they will part with the floor boot/bezel assembly (I tried a search on there for the shifter boot, but their search engine doesn't seem to be specific enough to bring anything up, so yeah, a phone call may be needed).
