Here’s a picture of the input shaft, are those lines anything to worry about? (the orange on the spline is grease, not rust)

The clutch kit is a clutchMax OEM 1995-2011 kit off ebay. Slave is also an ebay replacement that says “1995 - 1999”. The original pressure plate was a self-adjusting style and I replaced it with a non-adjusting one, though I have had the same issue with both pressure plates in the vehicle.

Try spinning the throw out bearing by hand. It should be smooth with little to no resistance.

For the tip of the shaft, clean it up with emory cloth.

Did you inspect every spline on the input shaft for a dimple or burr? It doesn’t take much to give the clutch plate something to hang up on.

While you have the slave cylinder out, try gravity bleeding it again, this time angle it so the bleeder valve is the highest point. That way any trapped air will be forced out.
