My belief is that it does based on a photo I saw of its underneath (and which I can't now find). Also that it has a balance shaft only on 4WD Rangers. Are these things so? Why should a balance shaft be present only on the 4WDs?
Where can I find some pictures/diagrams? (Better ones than the one in the Wikipedia article on the Cologne V6 which shows an old rusty engine.)

You asked a question then you answered it?

I believe it is only 3 throw. Ford likes to have two connecting rods per journal.

I have found no real pattern to which engines will have a balance shaft. I have taken these engine out of everything they are in and have found engines that should have had balance shafts that didn't, and ones that shouldn't have that did. The only way to be sure is to pull the front cover and look.
