I put a 3 inch lift on my truck last summer and had it aligned and camber set, but Im starting to have problems with the camber again after about 8k miles. It is acting opposite of what I keep finding online though. When I back up my tires stay pretty much straight up and down, maybe a little negative camber on the drivers side but not much. But when I drive forward both sides have about 1.5-3 degrees negative camber. Does this sound more like the springs are starting to sag, or a problem with the toe angle causing the camber to change?

An improper toe angle will cause an apparent change in camber (it's toed-out going by your description).

However if the angle of your steering linkage isn't lined up even with the axle, sagging springs can cause a toe-out condition to occur also, leading to this issue as well (and this may very well be the case since you say the camber goes from about straight to very negative, rather than it going equally negative and positive).

You might check where the ride height of your suspension sits, if it's much below where it should be then your springs are likely to blame (or maybe you simply need some washers as shims under the lower spring perches, something not uncommon with Supercab trucks, if that's what you have).

Info about ride height is near the bottom of this page:
